Comments: The home has four bedrooms which are all located on the main level. The home has a basement with laundry and storage area. The environmental center is located behind the house which makes it a wooded area. It is located a half mile from downtown Windom. The home is wheelchair accessible. Three of the individuals like to go tent camping, watching and feeding birds and a few of the individuals like to go to the movie theatre. All of the individuals like to go shopping, out to eat, and watching movies at home. All of the individuals work at the local day achievement center. One has additional community job four afternoons a week. Staff are trained in use of mechanical lift and have training in working with people with dementia related to Alzheimer like symptoms.

Comments: This home is set up like 2 apartments. Currently one apartment has 4 gentlemen and 1 lady living there. They enjoy going out to eat, movies, walks, shopping, concerts and attending ball games. The other apartment has 2 men and 4 ladies. They like to go out to eat, listen to music, and go on vacation. Both apartments are connected and staff is shared depending on the needs of the folks and the hours they work.

Comments: There are six bedrooms, a living room, kitchen, 2 baths, and a laundry room. The apartment is on the second floor and accessible by an elevator and a stairway. HCL is within walking distance from downtown Windom. There are two vans wheelchair accessible. HCL has been in existence for 30 years. The individuals living at this site range in age from 24-55 years of age. They often plan social activities including movies, going out to eat, swimming, concerts, shopping, vacation day/s etc. The ladies at this home are members of a Round Robin social group that meets monthly at different homes. Some of the gentleman are in a Men’s card club group that also meets monthly at a different home each month. Several of the individuals participate in the Special Olympics Spring Track and Field events. Staff are trained in medication administration, nebulizer treatments, suctioning, postural drainage, O2 administration, diabetes management, and carbohydrate counting.

Comments: There are six bedrooms on the main floor with two bathrooms. The laundry is also in the apartment. The bathrooms are both handicapped accessible with a whirlpool bath. The living room was newly remodeled and decorated. They live in a large 2 story brick building with 2 other apartments on the upper level of the building. They enjoy any outdoor activities such as fishing, parades, walks, picnics, and outdoor music concerts. The individuals living at this home are between the ages of 26 and 65. They enjoy playing a variety of games, cooking, watching movies. Some of the individuals belong to Peoples First, Special Olympics, Round Robin, and a card club. Staff are trained in CPR, First Aid, oxygen, suctioning, nebulizers, trach care, g-tube feedings, and medication administration.
