Person Centered Tools
Planning Alternative Tomorrow’s with Hope (PATH) is a person-centered planning tool using a graphic record to develop a visual map for an individual or team. Many individuals and organizations have used this process to assist them in planning possible and positive futures, solving tough situations, and helping get people unstuck from ruts. Is there another way to phrase this? - Helping people move forward with their lives.
Essential Lifestyle Planning (ELP) is a person-centered planning tool that uses trained facilitators. These plans provide the reader with a clear snapshot of how the person wants to live and what the reader needs to know or do to support the person.
Circles of Support is a group of people who agree to meet on a regular basis to help an individual accomplish personal visions or goals. The individual is unable to reach his/her goals alone, so he/she asks a number of people to join in and help overcome obstacles and open doors to new opportunities. Members of circles are usually friends, family members and sometimes service providers.
Personal Future Planning is an individualized, possibility based approach to life planning. A Personal Futures Plan represents a vision of a more desirable future. The plan is developed by a group of people who care about the individual and will assist in making the plan a reality. A facilitator helps organize a group to listen to the dreams and desires of a person and take action over time to accomplish those dreams.
Making Action Plans (MAPS) is a process that places primary emphasis on inclusion, participation, and learning in integrated settings. The process identifies action steps that move in the direction of inclusion in all aspects of life. A facilitator guides the process of discussion and planning.
